RN Medical Case Manager (Remote)

We are seeking for RN or LPN with previous case management experience. The hiring company would really love it if someone has Oncology experience. 

What to know: 

• 8:30 AM- 4:30 PM
• Monday to Friday
• 100% Remote
• $45 per hour 

Essential Functions: 

• Facilitating member involvement in suitable care plans, orchestrating care and services as necessary, communicating proficiently, and furnishing members with education and resources as required.
• Fostering member adherence to treatment plans, advocating for shared decision-making, and establishing relevant objectives to enhance member outcomes.
• Utilizing case management criteria, processes, policies, and regulatory standards to interpret, implement, and accurately document comprehensive care management plans.
• Establishing and sustaining member connections, actively engaging and maintaining member engagement to facilitate smooth care transitions and enhance health outcomes.
  

Qualifications: 

• At least 5+ yrs of RN experience with Care/Case Management experience.
• Active licensure in Massachusetts is required
• Oncology and Utilization Management experience, preferred.
• Proficient with multiple IT systems.
