Licensed Clinical Social Worker – up to $75/hour

 Job Summary:

We are seeking a highly qualified Case Manager with LICSW or LCSW certification to lead treatment planning and discharge coordination for youths with developmental disabilities. This role is integral to ensuring successful community reintegration and involves collaboration with interdisciplinary teams, families, and state agencies.

Key Responsibilities:

• Guide treatment teams in planning, care coordination, and transition processes.
• Conduct psychosocial assessments and pre-admission interviews.
• Evaluate outcomes and recommend changes to support individualized care goals.
• Liaise with state-run departments and local districts to ensure alignment in services.
• Provide patient and family education on adult services, insurance, and life skills.

Requirements:

• 3+ years of professional experience as a licensed social worker.
• Active LICSW or LCSW certification.
• Ability to work an on-site schedule in Canton, MA Monday-Friday 8am-430pm

Preferred Skills:

• Experience in youth transitions and hospital discharge planning.
• Social work experience with a youth population
