Case Manager – Addiction Recovery & Social Services

As a Case Manager, you will empower individuals in recovery by connecting them to essential services, advocating for their well-being, and supporting them on their journey to stability. This role offers a competitive pay of $35/hour.

Key Responsibilities:

• Develop personalized care plans to support addiction recovery and long-term well-being
• Serve as a trusted advocate, connecting individuals with housing, employment, and healthcare resources
• Collaborate with healthcare providers, counselors, and community organizations to provide holistic support
• Conduct assessments, track progress, and adjust care strategies as needed
• Maintain thorough documentation to ensure compliance with healthcare regulations
• Work flexible shifts to provide consistent support for those in need

Required Experience:

• Bachelor’s degree in Social Work, Human Services, Psychology, or a related field
• 1+ years of experience working with individuals affected by substance use, mental illness, addiction, or homelessness
• Flexibility to cover various shifts, including nights and weekends

Preferred Experience:

• Master’s in Social Work (MSW) or a Professional Social Work Certification (LICSW, LSW, LCSW)

Required Skills:

• Strong communication and interpersonal skills
• Excellent organizational and problem-solving abilities
• Proficiency in case management software and electronic medical records (a plus)
