Client Care Coordinator (Social Services)

Key Responsibilities:

• Provide direct support and case management for individuals in addiction recovery.
• Conduct client assessments, develop care plans, and track progress.
• Connect clients with essential services such as housing, employment, and healthcare.
• Coordinate with medical professionals, counselors, and community resources.
• Maintain accurate documentation and ensure compliance with regulatory guidelines.

Qualifications:

Required:

• Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
• 1+ years of experience supporting individuals with substance use, mental illness, or homelessness.
• Strong problem-solving, organizational, and communication skills.
• Flexibility to work variable shifts.

Preferred:

• Master’s in Social Work (MSW) or Social Work Certification (LICSW, LSW, LCSW).
• Experience using case management software and electronic medical records.
