Case Manager (Social Worker)

Responsibilities:

• Provide case management services for individuals in an addiction recovery facility.
• Conduct assessments, develop care plans, and monitor patient progress.
• Collaborate with healthcare providers, counselors, and community resources.
• Support patients with social service needs, including housing and employment assistance.
• Maintain accurate records and ensure compliance with relevant regulations.
• Work flexible shifts to provide continuous support as needed.

Required Qualifications:

• A Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
• 1+ years working with individuals experiencing substance use, mental illness, addiction, or homelessness
• Flexibility to cover various shifts, including nights and weekends.
• Strong communication, organizational, and problem-solving skills.
• Proficiency in case management software and electronic medical records is a plus.

Preferred Qualifications:

• A Master’s in Social Work (MSW) or a Professional Social Work Certification (LICSW, LSW, LCSW).
