Behavioral Health Case Manager 

We are seeking a dedicated Behavioral Health Case Manager to join our team. This role involves providing case management services for individuals in addiction recovery, supporting their journey to stability and independence. The ideal candidate will have a passion for working with individuals facing challenges such as substance use, mental illness, and homelessness.

What You’ll Do:

• Guide and Support – Conduct assessments, develop individualized care plans, and connect clients with essential services
• Advocate for Change – Help clients secure housing, employment, and access to healthcare
• Build Community Partnerships – Work closely with healthcare providers, counselors, and local organizations to ensure wraparound care
• Monitor Progress & Adapt – Track client growth, adjust support plans, and document everything to maintain compliance
• Be There When It Matters – Work flexible shifts, including evenings and weekends, to provide continuous care

Required Experience:

• 1+ years of experience working with individuals facing substance use, mental health challenges, or homelessness
• A Bachelor’s degree in Social Work, Human Services, Psychology, or a related field

Preferred Experience:

• A Master’s in Social Work (MSW) or a Professional Social Work Certification (LICSW, LSW, LCSW)
• Experience in crisis intervention, behavioral health counseling, or case management
• Knowledge of housing assistance programs and employment support services
