Case Management Pharmacist
We're hiring a clinical, member-facing Pharmacist to provide case management support for a well-known health insurance company. This is a high-touch role for someone who enjoys solving access-to-care and medication coverage challenges through strong clinical judgment, calm communication, and excellent follow-through and a great opportunity for an experienced clinical Pharmacist

- Pay Range: $55-$57/hour
- Job Type: 8-month contract
- Work arrangement: Mostly Remote, there are 2 days per month which require presence onsite in Hingham

This is a very clinical, non-retail pharmacist role where you'll spend your day directly supporting members who are often dealing with barriers to getting their medications (coverage, prior auth questions, cost, denials, benefit design). You'll make a tangible difference by listening carefully, investigating the situation, coordinating with providers, and guiding members through next steps--with empathy and confidence.
Responsibilities
- Serve as a primary clinical resource for members navigating medication access and benefit coverage questions.
- Conduct member outreach and inbound/outbound calls: gather clinical and coverage context, set expectations, and drive issues to resolution.
- Collaborate closely with providers (and other internal partners) to align on clinically appropriate care plans and next steps.
- Participate in weekly rounds, and present cases as needed.
- Manage and prioritize a dynamic daily workload (often ~10 active cases plus new referrals throughout the day).
- Document thoroughly and consistently in the team's tools (you'll be trained), including writing clear patient notes and tracking follow-ups.
- Use shared tracking resources (including a shared Excel file) carefully and accurately.
Credentials
- Active MA Pharmacist License (required)
Required Qualifications
- 1+ year of professional and recent experience in a clinical, patient/member-facing pharmacist role (not retail)
- Candidates will need to have used your clinical skillset in the real world within the last 5 years in order to be considered
- Comfort operating in a case-management / access-to-care environment: coverage issues, denials, affordability, benefit design, and coordination.
- Strong soft skills for difficult conversations:
- Active listening
- Empathy
- Ability to stay calm with upset members and keep calls structured (cannot skip steps)
- Excellent time management and prioritization (days change quickly; you can triage without getting rattled).
- Strong documentation habits and baseline computer proficiency (able to write clear notes; tools will be trained).
Preferred Qualifications
- Background aligned with ambulatory care, managed care/Medicare, hospital-based clinical coordination, or long-term care with strong provider collaboration.
- Experience working in an environment where you are able to communicate clinical thinking clearly to both clinicians and non-clinicians (ops, billing, etc.).
