Shift
- Training is from Monday to Friday, 9:00 AM – 5:00 PM
- Onsite training, with flexible scheduling afterward

Responsibilities
- Visit patients at home to assess health, safety, and daily living needs
- Determine eligible services such as home health aides or therapy programs
- Coordinate care plans with healthcare providers and community organizations
- Communicate effectively with families regarding care plans and requirements
- Maintain detailed records of assessments and interventions
Credentials
- Valid RN license in Massachusetts
Required Experience and Skills
- Minimum of 1 year of nursing experience
- Community nursing experience
- Interest in working with elderly populations
- Strong organization, communication, and clinical judgment skills
- Ability to work independently and coordinate care effectively
