Key Details:
- Direct hire with full-time benefits
- Pay Range of $90,000 - 93,600 plus a $3,000 sign-on bonus
- More flexible schedule after training, with in-home visits around the city of Boston
- A typical week will have 6-8 visits
- First six months are onsite for training Monday-Friday 9a-5p

Responsibilities
- Visit patients at home to assess health, safety, and daily living needs
- Determine eligible services, such as home health aides or therapy programs
- Coordinate care plans with other healthcare professionals and community resources
- Communicate effectively with families about care plans and requirements
- Maintain detailed records of assessments and interventions
Credentials
- Valid RN license in Massachusetts
Required Experience and Skills
- Minimum of 1 year of nursing experience
- Community nursing experience
- Interest in working with elderly populations
- Strong organization, communication, and clinical judgment skills
- Ability to work independently and coordinate care effectively
